FinalForms Registration Now Open for Lakeland Athletics!
All students and parents must create their own accounts on FinalForms. Once logged in:
Complete and sign all required forms on your profile.
Upload a current sports physical to your student’s account.
If you played a sport in the fall and still have an active sports physical, please upload a Health History Questionnaire Form to your profile instead.
Important: No paper packets will be accepted. All forms and uploads must be completed online through FinalForms.
Both the student and parent must sign all forms before participation in any athletic activity.
